This is a 65-year-old patient with a history of coronary artery disease, who is affected by a Left ventricular pseudoaneurysm (LVP)
The posterior wall of LV was opened over the pseudoaneurysm area and an organized thrombus, which was sealing off the rupture site, was removed.
Double patch of bovine pericardium was prepared. Interrupted 2-0 Ticron pledegtes mattress sutures were placed around the neck of the pseudoaneurysm then passed through the double-layered patch and tied down. Additionally, an additional procedure was performed to reinforce the closure of the pseudoaneurysm using 4.0 polypropylene sutures. This was done to ensure a complete and secure seal in the neck region of the pseudoaneurysm. The wall of the pseudoaneurysm was closed over the patch with a 3-0 polypropylene running technique using two Teflon felts around the suture line. In addition, the patient underwent bypass grafting of left anterior descending artery using Left Internal Mammary Artery.
Postoperative course was uneventful, without any complications.
