Rules:
1. Break the cover on turn one.
2. Do not use any vellumentals.
3. Do not use a 1-Up Mushroom / Life Shroom.
4. Only use the basic hammer attack.
5. Do not use any 2x attacks or 2x damage multiplier panels.
This basically guarantees that you have to dodge multiple of all four versions of the final cut (standard, follow, fake-out, and instant).
