A SLAP Lesion of the shoulder can be a tough condition to manage for most healthcare professionals. The purpose of this article to give patients a general understanding of the aspects of the condition we must manage and the reasons why it can take longer to resolve than some other sports injuries. Here are the reasons why:
1. It is a structural problem in the cartilage of the shoulder
2. Standard active rehabilitation and passive modalities have almost no affect on fixing the damage to the cartilage (it can help with shoulder function and pain)
3. Surgery is not always recommended
Let's start with what a SLAP Lesion. A SLAP Lesion is a condition where damage has occurred to a portion of the fibrocartilaginous rim around the glenoid fossa of the scapula/ shoulder blade. This rim allows for normal biomechanics of the humeral head on the glenoid fossa. With disruption of the rim comes pain, instability and altered biomechanics of the shoulder leading to a cascade of soft tissue inflammation through compensatory motion. A SLAP Lesion mean "Superior Labral tear Anterior to Posterior."
What is the cause of a SLAP Lesion?
There are a few different mechanisms of injury for a SLAP Lesion, but in sports it is most commonly found in overhead throwing athletes. Poor mechanics in throwing can be a major risk factor. Knowing what attaches to the portion of the labrum in the vicinity of a SLAP Lesion gives us insight to what happens. The tendon of the biceps brachii long head attaches to the labrum and when repetitive stress of "pulling" on the superior labrum occurs compromise and tearing will follow. Sound reasoning says we should just reattach the labrum but with any surgical intervention you must understand risk vs. reward.
What about physical or "physio" therapy? (Sorry I could not resist the semantics)
Some studies state therapy can help return players to their respective sports. But sadly depending upon the severity each athlete will respond differently. In 2010 a study indicated a 71% success rate in allowing athletes to return to participation levels after rehabilitation however only 66% where able to perform at the level prior to injury or higher. It is important to note in this study 51% of the "success group" opted for surgery after a "successful rehab." (Edwards, 2010)
