Complete spinal cord injuries
patient will develop complete motor and sensory loss below the level of the lesion with the return of the bulbocavernosus reflux. An injury above C4 will lead to paralysis of the respiratory muscles and respiratory support will be required.
Steroid protocol
Methylprednisolone 30 mg/kg should be given to patient with acute spinal cord injury followed by 5.4 mg/kg/hr for
•24 hours within 3 hours of injury
•48 hours within 3-8 hours of injury
Methyl prednisolone has no value if given after 8 hours following injury.
Steroid are becoming controversial. Patient may have complications with steroid use including GI bleeding. For now, some people believe that there is benefit from giving steroids within 8 hours of the injury.
