The save error codec calls were supposed to appear after the game would fail to save the game in the save game screen, however because of how the save menu works, once a error pops up the game doesn't jump right back to the codec screen, instead it jumps back to the Memory Card slot selection and if you quit from there, Otacon\Rose will act as if you just decided to not save, it's kinda the same situation for the format codec calls, interestingly enough, Rose has no format error codec calls and just one error codec call, unlike Otacon.
I've found those while disassembling the GCL scripts, I modified the "DO NOT SAVE" script to make them pop up.
