The patient has low back pain consistent with his imaging studies showing a grade 2 mobile L5-S1 spondylolisthesis. Pain is unrelenting despite exercise, activity modification, physical therapy, and other non-operative measures. Although his pain has improved, he is concerned with recurrent spontaneous flare-ups.
