This is something that is never taught to medical students or residents: how to read MRI’s, X-rays and CT scans. You absorb this information by looking at the films every day, but no one has broken it down to “this is what you find, this is why it occurs, and this is what can cause this injury.”
X-rays are the poor man’s MRI. There are a lot of doctors that do evaluations of patients and go straight to the MRI and that is a HUGE MISTAKE! The reason is because you do not know what is going on with gravity. If someone is getting an MRI, you are laying down; gravity pulls everything back into alignment. When you get to the point that you start to look at MRIs and then you look at X-rays, they are different. There is a shift, an angulation, a collapse. You ONLY see that with X-rays. You do not see that with MRIs.
What is black on an MRI? Bone, tendon, ligament. All those three things are black on an MRI. If you have an x-ray, it is like a CT scan in some respects even though you don’t see specific images, you can see with the bone equality in what’s happening. If you learn to put these together, it is important as a spine surgeon, and ultimately, for the patient.
