Medication errors (including wrong drug, dose, patient, route of administration and documentation) are major causes of inpatient morbidity and mortality. 1 in 20 perioperative medication administrations, or every second operation, resulted in a medication error and/or an adverse drug event (Nanji et al, 2016). Panelists at the Inaugural Patient Safety, Science & Technology Summit, Joshua S. Adler, MD; Richard C. Boothman; David C. Cassen, MD, MS; Michael R. Cohen, RPh, MS, ScD (hon.), DPS (hon.); M. Narendra Kini, MD; and Nathaniel Sims, MD, discuss adverse drug events – medication errors that cause harm to patients. The panel's consensus is that successfully adopting and implementing both healthcare information technologies and non-technology approaches is critical to improving and maintaining medication safety.
