old age lady admitted with sudden onset breathlessness , bp drop , saturation drop, tachycardia, ,2 D echo - Dilated RA, RV , free RV WALL HYPOKINESIA, severe PAH .
CTPA - massive bilateral pulmonary embolism..
thrombolysis with inj alteplase 100 mg ,
DVT present.

after thrombolysis CTPA done(12 hrs ) new thrombus in the main pulmonary artery, with partial improvement in the RPA & LPA thrombus, thrombus aspiration done using catheter..
large amount of thrombus aspiration..from bilateral pulmonary artery..
next day patient off oxygen..
discharge on next day ..
