Phacomorphic Glaucoma is type of angle closure glaucoma.
A enlarging cataract lens causes pupil block and anterior bowing of the iris.
Features:
High Intraocular pressure (67 in this case!)
Shallow AC
Mid/dilated fixed pupil
Swollen cataractous lens
We treated this patient medically first to get the pressure down - then listed for urgent surgery to removed the white cataract.
A very challenging surgery but the only way to bring the pressure down in the long term.
