You all must have heard about heart attacks, but did you know that there is also an attack that occurs in the eyes, which we refer to as an eye attack or, in medical terms, CRAO?
Hello, my name is Dr. Shreya Nayak, and I am a Retina Specialist. I work at AcuraVision Clinics Roop Netralaya in Meerut. The retina is a very important part of our eye, directly connected to the brain. Just as the diseases that cause heart attacks can affect the heart, they can similarly cause an eye attack. CRAO stands for Central Retinal Artery Occlusion. In this condition, the arteries in the eye become blocked, reducing blood flow, which results in an eye attack.
This is a very time-sensitive issue. Just as with a heart attack, if treatment is not given within a certain time, the person may not survive; similarly, in the case of CRAO, if treatment is not administered within 6-12 hours, it becomes very difficult to save the vision of the affected eye, and the patient may become completely blind.
The most common sign of an eye attack is the sudden onset of painless blindness, where you suddenly can't see anything without experiencing any pain, and everything turns black. This indicates that the blood vessels in the eye have been blocked.
This condition is more common in people with high blood pressure, diabetes, or uncontrolled cholesterol, similar to heart attacks. It has often been observed that stress or anger, which can trigger a heart attack, can also trigger an eye attack. If a patient comes to us within 6 hours of developing CRAO, we can perform certain treatments such as draining some fluid from the eye and massaging it to unblock the artery. If done within 6 hours, there are good chances of preserving some vision. However, if more than 6 hours have passed, it becomes very difficult to save any vision.
We conduct various tests to find the cause of CRAO, such as heart tests to check for thrombus, to prevent any other blockages that could affect the other eye. This is why CRAO is considered an emergency treatment for the eyes.
I had a patient who came to me three days ago with a problem of CRAO that had developed just 2 hours before. His vision was 1/60, meaning he could only see 1 meter away, whereas normal vision is 60 meters. We immediately performed procedures like paracentesis and digital massage. Miraculously, his vision improved to 6/36, which was a significant improvement in a case of CRAO. Both the patient and we were very pleased with the results.
However, the emergency treatment was just the beginning. Finding the cause was crucial. The patient was referred to a cardiologist who conducted an ECHO and ECG, discovering a blockage in the left side of the heart, which had likely caused the CRAO. This diagnosis was vital in preventing further complications.
Retinal problems like CRAO are often linked to systemic diseases such as high blood pressure and diabetes. Proper diagnosis and timely treatment can save not only the vision but also lives. Therefore, I urge everyone to take sudden vision loss seriously and consult an ophthalmologist, especially a retina specialist, immediately.
