Iki Island is the new DLC region in Ghost of Tsushima. You must have reached Region 2: Toyotama in the main game to access it. After installing the DLC you get a new Journal Entry in the pause menu and can track the first quest from there. It’s a relatively small region, about one third the size of a main game region (about a tenth of the main game map).
