How did the USS Stark, a strong US Navy ship, get hit by Iraqi Exocet anti-ship missiles?

Why didn't its advanced sensors and weapons stop the attack? Could this have been avoided? If yes, what caused the Stark to almost sink in the Persian Gulf?
