Idiopathic telangiectasias of the face (also known as facial spider veins thread veins or broken veins) are very common. They are very disfiguring and unsightly but can be treated very simply using electro-coagulation by shortwave diathermy. They are usually caused by sun exposure but other important causes include rosacea, liver disease, excess alcohol consumption, smoking and the use of topical steroid creams. I use the Silhouette Dermalift shortwave diathermy machine. The electrical coagulation current is applied directly to the thread vein using a very fine, single use, sterile needle. Before the treatment, the skin is numbed using a topical local anaesthetic cream which is applied 30 to 60 min in advance of the treatment session. The electrical current is applied to the thread vein via the needle using the correct electrical current setting and the correct pressure on the needle. As the current is applied, you should see the vessel blanche. If there is insufficient pressure or insufficient electrical current, the blood vessel will not blanche. It is important to use the point of the needle and apply the needle very accurately along the length of the blood vessel at 1 to 2 mm intervals. The blood vessel should only be treated once in a single session. It is important not to go over the thread veins more than once. If the needle is applied with excessive pressure and if the needle punctures the skin too deeply, there will be bleeding when the needle is removed. Over treating the veins should be avoided. Excessive electrical current or leaving the needle in one place for too long can damage the underlying tissues leading to pitting or scarring. It is better to err on the side of under treating in the first instance. in my practice I use 22-27 microamperes of current. If the 2nd treatment is required, it should be deferred for at least 6 weeks. After successful treatment the area should not be washed with soap and water for 72 hours. It is preferable to use a gentle cleanser and moisturiser. The treated area should be protected from sunlight and UV irradiation using a sunblock for 2 weeks.
