"There are four S's in Altered Mental Status"
"If it's NOT one of the 4's S's try out the NOT mnemonic"

Inspired by John Bielinski, I created my own mnemonic based on working up altered mental status patients.

SSSS-NOT

Sugar (Check a blood glucose level)
Stroke (Perform a Stroke Scale)
Seizures (Any "seizure" medications or signs of postictal state?)
Sepsis ( Any signs of recent illness or infection?)

If it's NOT one of the S's try NOT

Narcan ( Are we at an overdose of any kind?)
Oxygen (Is your patient hypoxic, check the SP02)
Trauma (Expose your patient, any sneaky trauma?)

Of course you would follow up with vital signs and an EKG and re-assess the patient as Vital signs and EKG may show the reason for this AMS state.
