The Red Cyberdemon from Map 23 of Brutal DOOM's Campaign Mod: Extermination Day, is probably one of, if not the Hardest Cyberdemon Boss Fight, and here it's made even more difficult with the Brutal Realism Difficulty which is Brutal DOOM's Hardest!

To be honest, although this can be considered the Hardest Cyberdemon, the boss itself isn't too bad, even with all that though, just like any Cyberdemon in pretty much any DOOM, just circle strafe it and you win lol, but also don't get hit like at all since Brutal Realism means 700% more damage is dealt to you.
