Nazareth Hospital provides comprehensive services in stroke care and serves Northeast Philadelphia and the surrounding region. Our stroke teams--comprised of board certified physicians, nurses, therapists and social workers--offer an integrated approach to diagnosis, treatment, and prevention of and education about stroke.

Mercy Home Health's Telehealth Program offers in-home telemonitoring to help patients from being readmitted to the hospital. Mercy nurses visit patients a few days during the week in their home and the patients take their own vital signs which are recorded and sent via telephone line to Mercy Home Health's command center.
