This is the loot cube puzzle easter egg / secret on the planet Nekrotafeyo towards the end of the Borderlands 3 storyline. This can get you some pretty sweet legendary loot but can only be done once per character, per playthrough and the loot rewards you get are totally random. I recommend activating Mayhem mode 3 aboard Sanctuary 3 before doing this easter egg to maximise your loot chances and also reaching a high level so that your loot will be decent to use for quite some time.
