This video depicts the surgical management of a patient who had high myopia and a central dense grade 3 Nucleus Opalescence (NO). The peripheral lens was clear. In this case certain modifications need to be made to the direct chop technique. Also one should adopt low flow parameters to reduce the risk of infusion deviation Syndrome (IDS) and Lens Iris Diaphragm retropulsion syndrome (LIDRS).
