Initial management:
Intensive monitoring with frequent neurologic examinations
Strict blood pressure (BP) control:
Keep BP below 180/105 mm Hg to ↓ risk of hemorrhagic transformation
Avoid hypotension to maintain perfusion to ischemic penumbra
Wait ≥24 hr prior to initiating antiplatelet therapy or anticoagulation
At some centers, a noncontrast CT scan of head is also obtained to first rule out hemorrhagic transformation
Potential complications:
Intracerebral hemorrhage
Systemic bleeding (eg, gastrointestinal hemorrhage)
Bradykinin-mediated angioedema
