Evidence-based care coordination allows for seamless transitions across the health care continuum in a deliberate effort to improve outcomes and reduce errors and redundancies. This overview is designed to offer best-practice tools and a roadmap for new and emerging care coordination models. Learn about care coordination implementation in a variety of care settings and programs, from what is being tried, tested, and applied by those on the care coordination journey. Case examples representing a range of programs demonstrate care coordination successes.
