Before anti-VEGF's, neovascular glaucoma would often present with a completely closed angle and a wide ciliary sulcus due to anterior contraction of the neovascular membrane on the iris surface. While tube implantation was often the definitive treatment, a Baerveldt 350 Glaucoma Implant could result in chronic hypotony. With anti-VEGF treatment, neovacular glaucoma has changed, patients presenting with open angles, visible angle neovascularisation and much less sick eyes. Now, an Ahmed Valve may not be enough as these eyes are much more robust and the Baerveldt Implant is more likely to be required for pressure control.
