If the family and the patient sees you doing your best for your patient, if they see that you are coming in twice a day when the patient is critical or three, four or five times a day, particularly when the patient is not doing well; if at that time they see that you are doing your very best, at the same time they can see that the nurses are doing their very best the resident doctors are doing their very best and if you are explaining to the patient's family at each stage that this is what is happening I think this is correct. I can't agree with Dr.Pandya more; I deal with with very sick patients in ICU all the time and they sometimes see great concern on my face and they come and tell me "Doctor you look very concerned, we know you're doing your best for the patient". They themselves realize that you are doing your very best. Its very important that all the residents and all the senior nurses as well. So its the team approach of care and concern. You can't know everything in medicine; that's not possible. Medicine is so unpredictable that all you can do is try your very best...do best as you can... and sometimes it a problem. There are many times I find that I'm at sea ! I don't know what's exactly wrong with him ! I know that this is wrong, this is wrong, but what's underlying reason for his illness may be very difficult for me to find out . That's not with me alone, I promise you that! Its with many people and people don't realize that; public also don't realize how some doctors...many doctors... in their course of duty...in their course of looking after patients suffer this emotional situation. Doctor shows a remarkable equanimity in the face towards the patient when he is examining the patient; when he is talking to the relatives; but hardly very few of them will know the concern. Very few realize sometimes the turmoil that goes on within. You may not want to say that he will not get well! At the same time you can't promise that they will get well! They will hold you responsible for what you said and then something went wrong.. that means that there must have been an error! So what I would say is that 'Most of such children will get well, but it depends on multiple variables.' One of the fails another comes first. One who fails, the parents can't go and blame the teacher for having done wrong. It depends on how every child responds and I won't know how this child is going to respond... but my experience says that majority respond favourably so I hope that your child will also respond favourably but we will keep a very close watch and if things are not going well we will certainly take an appropriate action. Which means that you've not said that he will do well....you have not said he's not going to recover... how can you say that? So telling them that everything is not in my hands but what is to be done best is in my hands but the outcome is clearly in not my hands but I expect a good outcome most of the time.
