In this video, Dr. Parsa Zadeh goes into depth about the anatomy of the TMJ and why pain will occur in the jaw. The condyle of lower jaw is suspended from the temporal bone by a capsule. Inside the capsule is a disc that moves with the condyle forward. The disc is very low in vascularity and what it gets is from the tissues called retrodiscal tissues. If the disc slips forward and stays, then the tissues at the end of the disc will get stretched. When the condyle moves up a bit then you will get pressure on those tissues, and that is where the pain comes from. The capsule does not sense pain and the front part of the disc is made not to sense pain either. It is the the back of the disc where nature did not design to withstand pain so it channels the nerve endings and vascularity through it. So if there is a little bit of space and the disc is forward that is when it becomes a painful situation.
A healthy person has equal space for the condyle. But if the disc moves forward and the condyle puts pressure on the tissue then it becomes a painful situation and TMJ pain arrives. An important question that is asked is, “What is the treatment for TMJ?” And the answer that can satisfy 99.9% of people is to get a splint. A splint is a night guard that has certain features embedded in it to take advantage of the natural and physiological feedback that exists on the teeth to minimize the pressure on the TMJ. A splint is also used to give negative feedback for the muscles of mastication to relax when the person starts to grind their teeth.
There are several types of splints although typically it is repositioning splints. Experienced clinicians can diagnose the need for anterior positioning splints, which are made and used when pressures on tissue and splint needs to be made a certain way The best thing that you can do for yourself is to get a splint made by an experienced clinician. So if you find yourself asking, “What will help with grinding my teeth?” Then you should want to get protection for your joints long term and you want to take advantage of the physiological feedback which are for your teeth. And the answer to do that is to get a splint for prevention of TMJ disease
