The transition from paediatric to adult services is frequently a difficult time for young people, their families and carers. The new clinical teams often don’t have the flexibility and responsiveness that paediatric services can or might lack in-depth knowledge of a young person’s complex condition. It can also cause considerable workload, headaches and last-minute panic for the primary care team when services that have been in place for years are suddenly not available anymore. This is particularly true for young people with life limiting and complex conditions, who often have particularly complex care needs.
This webinar hosted by the Royal College of General Practitioners and ‘Together for Short Lives’ will introduce new eLearning on this complex subject matter and discuss how to improve the transition journey for young people and their families with the help of the general practice team, resulting in a more streamlined workload for primary care teams and a better experience for their patients.
