A failing ABS Actuator on Toyota Hybrid models is very common issue. The ABS unit needs to be replaced and it's not cheap. I did the replacement on mine relatively very cheap because I bought a used module for $50! and it works! The process of bleeding the unit after the install is very simple and straightforward, just follow the instructions on the scan tool (required)
