The anterior drawer test of the ankle assesses for a lateral ankle sprain, specifically the anterior talofibular ligament, as well as ankle instability or hypermobility. The patient is seated with feet hanging off the table. The examiner places the heel in the palm of their hand with 10-15 degrees of plantarflexion. The examiner then translates the calcaneus anteriorly. A positive test is increased translation compared to the healthy ankle. The result can be quantified on a scale of 0 (indicating no laxity) to 3 (indicating gross laxity).
