There's also a program called a Chronic Special Needs Plan (C-SNPS) and that tends to be an HMO as well, but a chronic special needs plan is an individual that has a chronic special need, such as diabetes, cardiovascular disease, COPD or chronic obstructive pulmonary disease, and chronic asthma.

And under those conditions, the insurance companies have a form you fill out that states that you answer some medical questions and verify that you have been told by a doctor that those conditions exist. Also, you disclose any medication that you're taking that validates that those conditions are present.

When a doctor signs off on that the information is true you are qualified for that particular plan. After you qualify a Medicare Specialist will sign you up for the right special needs plan to fit your medical needs.
