The shoulder shrug sign is often seen in those with rotator cuff tears. When trying to actively elevate the arm, the person is unable and often shrugs with the scapula and upper trap as the humeral head migrates superiorly.
One way I Iike to examine a rotator cuff tear is to then actively elevate the person overhead and see if they can elevate actively themselves near end range. In this position it’s easier for the deltoid to assist the cuff to stabilize the humeral head.
