This takes place after The Siege of the Skaal Village, if you choose to become a werewolf. You must wait 3 days without curing Sanies Lupinus, then rest.

Make SURE there are no NPC near you whenever you transform. If they see you, the entire province becomes hostile to you.

When you transform the second time, Hircine will tell you to go to the Tombs of Skaalara to protect the Totem of Claw and Fang. Go there and kill every Skaal in the tomb.
