Topic : Finding MB2 canal in maxillary first molar .
🦷 Maxillary first molars have 3 roots and 4 canals
Mesiobuccal 1 - MB1
Mesiobuccal 2- MB2
Distobuccal - DB
Palatal - P
MB 2 canal :
🤔 Existence of canal
❓- Often debated as extra canal or Primary canal.
🔬According to various studies and advent of microscopes and CBCT they are present in almost 90 % of cases .
🦷Difficult to find ,missed and reason for failure of root canal treatment.
🎯Locating the second mesiobuccal canal in Maxillary first molar!
🤔How to locate MB2?
👉Analyse the preoperative radiograph
👉 Usual location: between MB1 and Palatal canal
👉 Rule of 2:
2 mm apical to the floor of pulp chamber and 2 mm away from MB1 (in mesiopalatal direction) [However, the Range is 1.1–4.1 mm] (Kewalramani R et al; 2019)
👉 White line test (Nonvital tooth) and Red line test (Vital tooth)
PS: Don't violate the endodontic dentinal map in order to find the canal.
Do it in 2 Stages:
⏳Stage 1 : After the trapezoidal access opening, scout, prepare and irrigate all the three conventional canals MB1 , DB and Palatal canal without attempting to find MB2 during the procedure....
⌛ Stage 2: Now after preparing the conventional canals MB1, DB and P visualise the prepared orifices ...
📌DG-16 Probe or the micro- openers comes in handy for locating the canal.
📌Walk your probe on an imaginary line drawn from MB1 to Palatal.
🎯Usually one finds MB2 at 2/3rd the distance between Palatal and MB1.
📌Ultrasonic tips are also useful in locating and troughing the canal
📌Used in low power with less water
🔍Magnification definitely helps in locating the canal with ease.
⏳It takes a lot of time, patience and practice to find the canal...
