This video provides information on the format and concepts presented in Chronic Disease Self-Management Education (CDSME) programs. These group-based programs meet for a set time each week for six weeks and can be provided remotely, in-person or by telephone. There are a variety of different program types that focus on various conditions (e.g., diabetes) or that were designed with cultural modifications. All programs are evidence-based, demonstrated by multiple research studies and follow a curriculum developed by Stanford University to ensure required concepts are included and that all programs are provided consistently.
