Step-by-step description of what to do if the micropuncture dilator will not advance after insertion of a 0.018 inch guidewire:
1. Scalpel to enlarge skin entry point
2. Use a new dilator
3. Use the dilator inner sheath + Platinum Plus wire
4. Use a radial sheath
5. Manual pressure + repeat puncture

If kinking: could cut the wire and use a short dilator segment to enter the vessel
