This is a highly myopic eye with a congenital coloboma of the iris. We are going to do phaco with IOL placement here and then fix the iris coloboma using a variation of a technique that I first saw presented by Bobby Cionni. This technique creates flaps that can be brought together to create a centered pupil rather than one that is displaced inferiorly in the direction of the coloboma which is what one would get if they simply closed the defect directly.
