This video demonstrates how to perform an abdominal aortic aneurysm (AAA) ultrasound using color Doppler and measuring the short axis view.

In this video, you'll find an 86 year-old patient positioned in a supine position. The ultrasound transducer is taken with the marker to the right-hand side of the patient. First you see the short axis view and then the transducer is moved downwards more caudally. An abdominal aortic aneurysm (AAA) comes into view, and on the left-hand side of the screen, you can see the vena cava. Down in the lower part of the image, you can see that the vertebral spine seems to be very much echogenic, like the AAA in this case.
When the transducer movement goes from cranial to caudal, the AAA appears with a slight curve to the left side, until going back to the midline and to the navel area towards the bifurcation. A color Doppler is then performed with this already in transverse. By moving in the direction of the AAA, you can see the color Doppler information only in the lumen and not where you see the thrombotic material. Going more caudally, you can see the smaller lumen of the abdominal aorta.
Next, the same process is undertaken in the longitudinal view. The midline of the patient is found, and you see again that this AAA is in a slight S-curve. You can see the lumen of the AAA. Again, apply color Doppler to distinguish the true lumen and the thrombotic material. You can move the transducer a little bit to the left and to the right. Turn off the color Doppler caudally to visualize where the AAA is actually located.
To finish the exam, a measurement is taken in the short axis view. Make sure that you are truly perpendicular to the location of the AAA when you take the measurement. Freeze the ultrasound image, and perform the measurement. This is really easy and fast to do. Place the markers on the screen. Perform the measurements from AP and also from the side. Store the image.
In summary, you can see how easy it is to perform an ultrasound to include or exclude an AAA and the presence of abdominal pain and symptoms.
