Case illustrates the limitations of angiography in assessing risk of post-percutaneous coronary intervention (PCI) occlusions. In two similar stenoses (based on angiography), presence of lipid core plaque, detected by near-infrared spectroscopy (NIRS), could indicate greater risk of future occlusions and inform the choice between PCI and bypass.
