This vignette illustrates common safety errors that can affect medication administration. To use this vignette, download the Pedi-CSI observation sheet and note all safety errors you see while watching the video, whether related to medication administration or not. Once complete, the vignette will repeat with each of the safety errors identified so you can check your observations.
About the Pedi-CSI (Clinical Safety Instruction) Patient Safety Program
This training program was developed as an outcome of an Academic-Practice Partnership between Boston Children’s Hospital, Department of Medicine Patient Services, and Boston College’s William F. Connell School of Nursing. It was designed to provide individuals teaching patient safety for or in new nurse orientation as a useful tool for communicating critical patient safety concepts in real patient care settings. This program consists of twelve vignettes of real patient care situations. Across the twelve vignettes, overt and subtle safety errors are staged.
