The success rate of pancreas transplant in type 2 diabetes is not so robust as kidney transplant. There are mainly two reasons for that. One is to get an appropriate donor is very tough and we have to get cadaveric donor and it is not possible to get live donor for pancreas as the patient who donates pancreas will develop diabetes. Once we have done transplant the patient has to be on immunosuppressants and it itself contributes developing diabetes in future. Hence pancreas transplant is restricted only to those who are on severe diabetes, difficult to control, on insulin and sometimes having fluctuating sugar levels, severe hypoglycaemia, hypoglycaemia unawareness with kidney failure. Those are the ideal candidates for pancreatic and kidney transplant together instead of doing pancreatic transplant alone. Definitely there is something called islet cell transplant where immunosuppressants are not required for long term. They show some response and usual response rate anywhere varies between 10 to 30 % as the success rate.
