First of all, I want to congratulate you on the birth of your baby boy. And it sounds like you are having a couple of issues especially with feeding. You're having a hard time getting him to burp. You've burped him for 45 minutes and he doesn't burp. But when you lay him down, everything comes back up out of his mouth and nose. And you've tried different kinds of formula. There are some basic principles to babies who are just extra spitty and don't actually have any medical condition behind the spitting up. So in general, here's a few things you can try. First of all, avoid overfeeding your baby and letting your baby eat too quickly. So if you're giving your baby a bottle, try stopping halfway through the feeding, and burp your baby halfway through the feeding instead of just at the end. So give your baby half a bottle, burp him - and you mentioned you've been burping for 45 minutes, and usually if babies don't burp after 10 to 15 minutes, they're not going to. So burp for 10 to 15 minutes in between the feeding, finish the second half, and then burp again. And try to keep him upright for 20 to 30 minutes afterwards, and you did mention you do keep him upright, but these are just some general principles that you can continue to try. And also, you might just want to try giving him smaller feedings more frequently throughout the day, and this might help reduce the chance that if his stomach is just getting overfilled, or it's getting filled too quickly, this will cause everything to come back up.
Also avoid any extra pressure on his tummy. So sometimes babies are wearing super tight pants, and that adds additional abdominal pressure that causes everything to come up. And don't put him on his tummy for tummy time immediately after a feeding. Give it some time, at least 30 minutes, and probably to be even safer, 60 minutes to make sure he doesn't throw his meal back up. In some cases it can also be formula. About 5% of babies have a true milk intolerance, and special formula types are necessary. You did mention that you switched formulas, and I wouldn't recommend switching to any other kind unless you're directed by your pediatrician. So the next step, and I think this would be best for you, would be to talk to your son's pediatrician, and they can decide if there's any medical reason behind your son's extra spittiness. Some babies have issues with reflux, and in some cases, medications are needed, or other treatments. But generally speaking, most babies do have a little bit of a spit-up issue because the little flap that goes over their tummy sometimes hasn't fully developed and it opens when it's not supposed to, and so it just causes everything to come up. It could be one of those cases, or it might require more extensive treatment, and your doctor can decide.
