Patients with gender dysphoria undergo several surgical procedures to achieve congruence with their identified gender. One of the most commonly performed procedures is the female to male mastectomy, also known as TOP surgery. It is a chest masculinising procedure. A variety have been described and the most commonly performed procedure is the double incision mastectomy with free nipple grafts We show one of our patients at 3 months post op who underwent a double incision mastectomy with free nipple grafts. We advise continuous care after the procedure with silicone sheets over the scars as well as wearing a compression garment at night time for up to 12 weeks after surgery. We have simplified this procedure to the extent where patients go home the same day after surgery and we do NOT use drains
