This is Darkspore a short-lived game by Maxis and a Diablo-esque spin-off of the Spore franchise. Unfortunately EA (the worst publisher) shut down the servers in 2016 making the game unplayable, due to the game requiring the player to be online in order to even run it.
Fortunately, some members of the community are working on making the game playable again and have got the hero creator and main menu working. This project is called Resurrection Capsule.
