CLINDAMYCIN:
Class:
Antibiotic --Lincosamide
Mechanism of Action:
Reversibly binds to 50s ribosomal subunits preventing peptide bond formulation, thus inhibiting bacterial protein synthesis.
Bacteriostatic at low concentration.
Bactericidal at very high dosage.
Clindamycin is a very useful antibiotic, but you must be very careful:
I.M must be Deep I.M, rotate sites and not greater than 600mg at once
I.V: Infuse slowly from 10-60 minutes
Rate, not greater than 30mg/min
Too fast can lead to a very severe hypotension and cardiopulmonary arrest
Never give the undiluted bolus
Never exceed 1, 200mg/hour
Clostridioides difficile leading to Fatal Colitis is a possibility.
To be reserved for serious infections

Dental procedure
GBS
Infective endocarditis
Hidradenitis suppurativa
Malaria
Neutropenic fever in cancer patients
PCP or P jiroveci Pneumonia
MRSA pneumonia
Rhinosinusitis
Prosthetic joint infection
Group A strep pharyngitis
Toxoplasmosis prevention
Strep pyogenes and aureus in skin infection
Septicemia by anaerobes
Enterococcus Faecalis or S aureus
Osteomyelitis
Endometritis
Non- gonococcal tubo – ovarian abscess
Pelvic cellulitis
Peritonitis
Anaerobic caused abdominal abscess
Pneumonia
Empyema thoracis
Lung abscess
In pregnancy:
Useful for prophylaxis against GBS when penicillin is C/I
Prophylaxis against Toxoplasmosis and Pneumocystis J/C pneumonia
Treatment of the 2 above i.e. Toxo and PJP
Pre-op antibiotics for C/S
Endocarditis prophylaxis in certain women during vaginal delivery
