Please submit the following to the Consulate (Within 30 Days) after notification/acceptance of marriage at the Ward Office/City Hall:
Four (4) duly accomplished forms for Report of Marriage(all forms must be typewritten or written legibly, original and signed).
Four (4) photocopies of valid passport (data page) of Filipino registrant
Four (4) photocopies of passport and/or Japanese government issued ID's of foreign spouse.
Kon-in Todoke Kisai Jiko Shoumeisho (one Certified True Copy and three photocopies)
If the spouse of the Filipino national is Japanese, one (1) original and three (3) photocopies of his Kosekitohon family register with details of the marriage.
If the spouse of the Filipino national is foreign but not Japanese, one (1) original and three (3) photocopies of Kon-in Todoke Juri Shomeisho
Self-addressed retum envelope : Letterpack 510
If the Report of Marriage is being filed beyond the reglementary period of thirty (30) days, submit an Affidavit of Delayed Registration of Marriage.
Fees:
Report of Marriage - 3,250 yen
Translation of Kosekitohon: 3,250 yen
Affidavit: 3,250 yen per document
Translation of Konin Todoke: 3,250 yen
Additional Requirements, where applicable (Applicants who have been issued an LCCM by the Consulate no longer need to submit these additional requirements if they have already previously submitted these documents to the Consulate):
One (1) copy of the PSA / NSO Birth Certificate of Live Birth or PSA/ NSO Report of Birth authenticated by the DFA (red ribbon)
For Filipino nationals who were previously married but have been divorced please submit:
One (1) original of the Philippine Judicial Recognition of Divorce with Certificate of Finality authenticated by the DFA (red ribbon)
One (1) original of the PSA / NSO Marriage Certificate or Report of Marriage with Annotation on the Judicial Recognition of Divorce authenticated by the DFA (red ribbon)
One original of the PSA/NSO Advisory on Marriage/s authenticated by the DFA (red ribbon)
For Filipino nationals who were previously married but had their marriage annulled, please submit:
One (1) original of the Philippine Judicial Recognition of nullity of marriage with Certificate of Finality authenticated by the DFA (red ribbon)
One (1) original of the PSA Marriage Certificate / Report of Marriage with the annotation on the nullity of marriage authenticated by the DFA (red ribbon)
One (1) original of the PSA/NSO Advisory on Marriage authenticated by the DFA (red ribbon)
For Filipino nationals who were previously married but have been widowed, please submit:
If death occurred in the Philippines, one (1) original of the PSA / NSO Death Certificate of deceased spouse authenticated by the DFA (red ribbon)
If death occurred in Japan and deceased spouse is Japanese, One (1) original of the Shibo todoke kisai jiko shomeisho and Kosekitohon
One (1) original of the PSA / NSO Marriage Contract or Report of Marriage authenticated by the DFA (red ribbon)
One (1) original of the PSA/NSO Advisory on Marriage authenticated by the DFA (red ribbon
