This is a facial contouring surgery done for giving a more feminine facial appearance and for a more attractive look. The surgical plan was to correct the excessively square-shaped face by trimming the bone in the mandibular angle region and the enlarged masseter, decreasing the frontal bossing and lowering the hairline and a reduction rhinoplasty for making the nose appear sleeker and slender.
A square-shaped face is considered a more masculine trait. To correct this, through an intraoral approach the buccal plate of mandible in the angle region on both sides were trimmed. The hypertrophied master muscle on both sides was excised. This combined gave the lower half of the face a more tapering, gentle appearance. A bicoronal flap was raised to expose the skull bone. The outer table of the frontal sinus was reduced to reduce the frontal bossing (excessive prominence of the forehead) making it more delicately shaped. To protect the frontal sinus, the site was covered with a Titanium mesh. By removing the excess skin, the hairline was lowered making the forehead appear less broad.
To complete the facial reconstruction to get a more feminine appearance, a reduction rhinoplasty was done. From inside the nose itself, the lower lateral nasal septal cartilages were trimmed on both sides to decrease the nasal tip and alar. A lateral nasal osteotomy was done to reduce the width of the dorsum, making the nose appear more sleek, slender and pristine. These procedures done together in a single-stage surgery, gave the face a more feminine, pleasing appearance.
