Spurling’s test is used to evaluate for cervical radiculopathy among other causes of neck and upper extremity pain. The patient is seated with the examiner in a position to apply axial load to the top of the patient’s head. The patient places their neck in extension with rotation in the direction of the patient’s symptoms. The examiner applies an axial load. This can be repeated with the head rotated to the unaffected sign. A positive test is reproduction of pain radiating into the affected upper extremity.
