In this video, Dr. Millett demonstrates an arthroscopic knotless double-row extended linked repair of massive posterosuperior rotator cuff tear. This patient is a 56 year old male with an acute massive rotator cuff tear involving the supraspinatus and the upper portion of the teres minor. This massive rotator cuff tear will require a double-row rotator cuff repair with an extended linked speed-bridge construct.
The arthroscopic rotator cuff repair uses 8 anchors, 4 medially and 4 laterally. The arthroscopic diagnostic confirms the massive rotator cuff tear. The greater tuberosity footprint is repaired using an arthroscopic shaver.
The initial anchor is placed anteriorly and the second anchor is placed posteriorly. Working from outside-in helps avoid the formation of dog ears. The suture loaded anchors are inserted into the humeral head and the sutures are then inserted through the rotator cuff tendon.
The rotator cuff is secured back to its original footprint. Physical therapy is started with early passive range of motion. At four weeks, the patient can begin active and active-assisted range of motion. Strengthening can begin at about 6-8 weeks.
