In this video, you will learn, again a very complicated term acute torticollis. It is also called as wry neck.
The term torticollis or wry neck is used to describe a condition, when there is asymmetry in the level, of neck and shoulder. The commonest muscle that gets involved is called as sternocleidomastoid muscle. This muscle is located in front of the neck region and is also responsible for movement like, rotation of neck, side bending and movement of neck region.
Symptoms associated with WRY NECK:
1. Pain in cervical spine
2.Tenderness in the neck region
3. Asymmetry of neck
4. Difficulty in the movement of neck.
5. Tremors in neck region.
6. Difficulty to move the shoulders.
7. Mass formation around neck.
8. Thickening of sternocleidomastoid muscle.
9. Unequal shoulder height.
Causes of Torticollis :
When we look at the causes of torticollis it can be divided in two parts.
1. Congenital torticollis ( torticollis since birth)
2. Acquired torticollis (acquired following an episode of disease or other kind of abnormality.
Congenital torticollis is usually seen in the kids who have injury to the neck muscles, following birthing process or because of mal- position of the head, in the mother's womb.This leads to the damage of sternocleidomastoid muscle, leading to the signs and symptoms of cervical spondylosis.
Next is an acquired torticollis, it is acquired following a disease or following an injury.
Common reasons of acquired torticollis:
1. Disease of cervical bone.
2. Following a cold episode
3. Staying in an unusual posture for long time.
4. Tumour of base of skull.
5.Throat infections.
6. Ear infection
7. Overuse of certain drugs (antipsychotics).
Acquired torticollis is seen in both adults as well as kids.
How it is diagnosed?
Diagnosis of torticollis, is really important. Because, once the cause is known well, the treatment can be planned accordingly.Hence it is really important, for you to visit to the doctor and get yourself diagnosed.
When you visit a doctor, the basic process starts with the
History taking, followed by physical examination,were the muscle are inspected and the strength of nerve is checked. Followed by this, a doctor may prescribe you some diagnostic procedure, like Xray, CTScan, MRI. Sometimes even EMG, which means electromyography.
Management of torticollis, basically depends on the cause.
When, the cause of torticollis, is because of muscular reason, due to some malpositioning or exertion, at that time physiotherapy is the main line of treatment. Rest of the time surgical management, and medical management is the line of treatment.
Medication used are like:
1. Muscle relaxant
2. Antidepressent
3. Non steroidal anti-inflammatory drugs
4. Corticosteroids
Surgical procedures are opted when the conservative management does not work:
After surgical process, as well intense physiotherapy is required to gain the mobility of sternocleidomastoid muscle.
When torticollis is since birth the methods like botulinum injections, muscle release technique is used in order to maintain the mobility of neck muscles.
As physiotherapy has got an major role in management of Torticollis, it needs and detailed approach.
