2.2kg baby , post dated , MSAF. Baby did not cry immediately after birth. Required bag and mask ventilation for 1 minute. At admission Downe's score of 5/10: chest Xray done at admission showed right pneumothorax. Baby was put on CPAP I/v/o respiratory distress with settings of 30 FiO2 , PEEP 5. Xray repeated 8hrs showed resolution of pneumothorax . But 6 hrs later , respiratory distress worsened. Baby was intubated. Chest Xray repeated showed re appearance of pneumothorax, ventilation was continued. With ventilation distress reduced.
You see clear cut presence of comet tail appearance, sliding lung , sea shore appearance in normal lung where as no sliding , absent comet tail, bar code sign in pneumothorax.
Lung point where both the features meet is clearly visible , and highly specific to pneumothorax
