✅ Uterine fibroids are a benign and self-limited disease, they grow thanks to estrogens, and therefore it is a disease that disappears spontaneously with the menopause
They vary in number, size, and location, but in almost all cases that require a surgical treatment we can treat them by minimally invasive laparoscopic surgery.
✅ This laparoscopic surgery is carried out through small incisions, one in the umbilicus of approximately one centimeter where we introduce a camera and three smaller incisions below in the pelvis of 5 millimeters, where we introduce surgical instruments to do the procedure.
✅ The pedunculated fibroids are usually removed before the placement of ligation with a surgical suture to break down the blood vessels at the base of that fibroid and then gradually removed from the uterine body with great care to control bleeding as much as possible.
✅ Regarding intramural fibroids, we use a substance that breaks down the blood vessels. In this case, fibroids of more than 4 centimeters but sometimes we operate on patients with fibroids of 10 or 12 centimeters through laparoscopic surgery.
✅ This substance breaks down the blood vessels to reduce intraoperative bleeding then with monopolar energy what we do is incise the uterine wall/ uterine muscle, which allows us to visualize this nodular structure which is the fibroid.
✅ We are going to separate this uterine fibroid delicately from the wall of the uterus and when we arrive at the base of the fibroid, we will break it down with bipolar energy of all the blood vessels that come from the base of the fibroid.
✅ The objective of this, is to reduce the bleeding until we remove it completely from the wall of the uterus. Next, we approximate the edges of the uterine muscle with a surgical suture.
✅ This is a very important part of the surgery to avoid bleeding during and after the surgery for which we use a special suture. And the objective here is to reconstruct the anatomy of the uterus as much as possible so that the woman can not only keep her uterus but also a uterus with the capacity for future fertility.
✅ Finally, we introduce a bag inside the abdomen through the umbilical orifice. And the purpose of this bag is to place the fibroids (previously removed) inside the bag and then remove this bag through the umbilicus. In this way we eliminate the fibroids from the patient's body.
