Base chance to hit: 40%
Highest chance to hit: 85%
Lowest chance to hit: 15%
Melee attack, defence and charge bonus can modify these values by 1 per unit of difference up to the highest and down to the lowest "to hit"-chance. Base chance is when the attack/defense balance eachother out, I assume. Warning, this video contains mental arithmetic done early in the morning and could contain inaccuracies.
Now I knew there was an upper an lower threshold for the chance to hit or get hit in melee combat - exactly what it was and how melee attack/defense modified it I did not know. With the help of a post from WalrusJones, now I think I do.
